HSSC — Access Medical Surveillance (Health Screening)

If directed to obtain Health Screening you will need to:

• Make an appointment with the Stanford University Occupational Health Center (SUOHC).
  Call 650-725-5308 to schedule an appointment. Inform the Center that you are a SU School of Medicine employee participating in the HSSC program with Spectrum OTC.
• Download, complete and bring with you to your appointment the following:
  
  1. Gather all supporting medical and immunization records, as available.-Please provide a copy of your immunization records to SUOHC.
  2. Tuberculosis Screening Questionnaire
  3. Hepatitis B Declaration Form
  4. Health Screening Payment Authorization Form (PAF). (Word doc) The PAF is crucial. Service will not be rendered without a complete form.
• Depending on one's immunity history and medical profile, SUOHC will determine what, if any, medical screening is required.
  The HSSC medical surveillance screening generally would include testing for tuberculosis, hepatitis B, measles, rubella and varicella (chickenpox). Not all employees will need all tests and immunizations.
• Once you have completed the screening process, you and your supervisor will receive an email declaring that you are “cleared, no restrictions” which indicates you are eligible to see research participants.

Medical Surveillance (Health Screening) is an annual requirement. All health records and medical information are protected health information and will reside with SUOHC. If you have questions regarding completion of health screening within the past 12 months, please contact HSSC Support
